Patient Services



    The providers at North Valley Family Medicine are devoted to providing quality, compassionate, comprehensive health care to our patients of all ages. Not only do we diagnose and treat medical illnesses, we also provide full service primary care medicine which includes:

  • Wellness and preventative examinations
  • Gynecological services
  • Well child examinations
  • Immunizations and screening tests
  • Comprehensive chronic disease management
  • Behavioral health
  • Skin cancer screenings
  • Minor skin procedures
  • Allergy skin testing and immunotherapy
  • Because allergies are such a common medical condition that many patients suffer from, we provide on-site skin testing and treatment plan unique to each patient’s needs. For more information regarding our allergy program, please ask your medical assistant or provider.

    We also offer diagnostic testing in the office which includes EKG, spirometry (lung function test), ABI (screening for vascular disease), rapid strep test, rapid flu test, urine analysis and pregnancy test.

General Information


 

Office Hours

Our office hours are Monday through Friday 8:00 am – 5:00 pm and Saturday 8:00 am- 12:00 pm. The office is open during the lunch hour.


Lab Draw Station Information

For the convenience of our patients, we have a phlebotomist in our office to perform blood draws. The phlebotomist performs blood draws Monday through Friday from 8:00 am – 12:30 pm and 1:30 pm - 4:30 pm. On Saturdays, lab draws are done from 8:00 am - 11:00 am.

Any lab billing questions should be directed to Sonora Quest and Lab Corp.


New Patients

Please arrive at least 15 minutes prior to your appointment time with your photo ID card and your medical insurance card. Also, please bring a list of your mediations and dosages.


Insurance and Payment Information

North Valley Family Medicine accepts most insurance plans. For specific information regarding your health care benefits or if you have additional questions, please contact our office.

We collect copayments when you check in for your appointment. We accept Visa, MasterCard, Discover, American Express, cash and checks with a driver’s license.


Appointment Information

We appreciate that your time is valuable and we make every effort to be punctual with our appointments. However, we strive to give each patient the time that is needed to address his/her medical issues. Unfortunately, our providers cannot always anticipate the needs of a patient until he/she has been evaluated, sometimes making it difficult for a provider to stay on time.


Appointment Cancellations

If you are more than 15 minutes late to your scheduled appointment, you may be asked to reschedule your appointment to another time or day.

If you are unable to keep your scheduled appointment, please contact the office as soon as possible. Appointment cancellations should be called into the office a minimum of 24 hours in advance as a courtesy to other patients. If you do not cancel your appointment, you may be assessed a fee.


After Hours

We have a provider on call after hours and on weekends for urgent matters. For these urgent matters after hours or on weekends, you can call our office phone number at (623) 322-4991 and you will be connected to our answering service. The answering service will then contact your provider. If you have a life threatening emergency, you should call 911 and go to the nearest emergency room.

Medication refills, antibiotic prescriptions or narcotic prescriptions are not done after hours.
